We want to know how germs pass between people.
To do this we want to know about you and the people that you met.
Many thanks for your help.
Next we are going to ask you some easy things about you:
My name is .
I am years old.
I am a Boy, Girl.

My school's name is .
I am filling this form in for Yesterday .
When we are ill it is often due to germs which are passed from person to person.
Using tissues when you sneeze and hand-washing can kill the germs and stop people getting ill.
Because germs are passed between people, we want to know the people you got close enough to talk to.
Think about your day, from getting up in the morning to going to bed, who did you meet and talk to?
In the boxes below please type the names of everyone you talked to. Do not worry about spelling the names right.
New boxes will be made as you need them.
Do not count phone calls or teachers talking to the whole class.
Some diseases spread in the air, some germs are only passed when you touch
This is why you should always wash your hands.
Now think about the people you touched, hugged or kissed
For each person, tick the box if you touched or not.
Did you touch?
Thnk about where you met the people you talked to:
At your home? At school? Or somewhere else?
Tick the boxes to show where you met each person.
Think about how long (in total) you spent with each person.
There are four choices:
Tick the box to show how long you spent with each person.
Groups of friends know each other and play together. All the people in a house know each other.
This is gives lots of ways that germs can spread.
For each person you talked to, we will show you a list of all the other people. Tick the box if you think the two people met on that day or in the week before.
For example:
On the day that you are telling us about, or in the last week:
Did Sam meet?
Move to person.
You have helped to fight germs. We will use all the information you've given us to make better models of how germs move between people and help us stop them from spreading.
